Hospitalisation Risk Algorithm addresses Australia’s healthcare crisis of
748,000 annual preventable hospitalisations costing over $2.4 billion. A
PenCS App module aids health providers in identifying at-risk patients;
ensures timely recalls to the General Practitioner, Aboriginal clinics and
preventive care, and reduces hospitalisations. The module was embraced
by eight Primary Health Networks leading to thousands of prevented
hospitalisations, offsetting a projected shortage of 9,298 GPs by
2030. By emphasizing preventive care over hospital treatment, Hospitalisation
Risk Algorithm aims to transform healthcare for a sustainable
future.
